Travel Health
A Saint Mary's College Travel Health Form (see below) is required for all SMC-sanctioned study abroad or Jan Term Travel. Completed forms must be provided to the Jan Term Travel office or Study Abroad office to enable travel.
The Student Health Center offers travel appointments to complete this form. A visit fee of $25 will be charged to your account for these appointments. Alternately, this form can be completed by any off-campus healthcare provider of your choice.
To Schedule your Travel Visit at the Student Health Center:

Please include the following in your email:
1) Subject heading of "Travel Visit"
2) Full name/SMC ID
3) Travel destination and dates
4) Phone number
One of our nurses will get back to you to review your immunizations. To expedite this process, please make sure documentation of your current immunization record (hopefully you uploaded this upon your enrollment!) has been uploaded to Med+Proctor. (If you entered SMC before Fall 2023, please make sure your record was uploaded to the Student Health Portal OR you can upload to Med+Proctor)
For travel paperwork to be approved by the SMC Student Health Center, all immunizations recommended by the CDC for your travel destination are required. We must take into consideration the health and experience of your entire travel cohort. Accordingly, for forms to be signed by the Student Health Center staff, ALL Jan Term travel destinations will require the following:
1) Routine childhood vaccines: MMR, Varicella, Polio, Tdap (within 10 years), Meningitis ACWY (after 16th birthday). See vaccine details here
2) Flu shot (available at the Student Health Center at your in-person visit)
3) Covid-19 (current season)
In addition to the above, the following will be required according to destination:
India and Philippines (3 additional) : 1) Hepatitis A (2 doses at least 6 months apart or 1 dose if travel is within next 6 months), 2) Typhoid vaccine (prescription will be given at in-person visit if needed), 3) Anti-Malarial medications (prescription will be given at visit).
Mexico, Costa Rica OR South Korea (2 additional) : 1) Hepatitis A (2 doses at least 6 months apart or 1 dose if travel is within next 6 months), 2) Typhoid vaccine (prescription will be given at in-person visit if needed)
Your in-person Travel Visit will be scheduled once it is determined your pre-visit immunization requirements have been met.
If anti-malarial medications or the typhoid vaccine are required for your destination, prescriptions will be given to you at your in-person travel visit. If a Yellow Fever vaccine is required for your destination, this may be obtained after your form is signed. (You can find vaccine clinics offering this here).
Scheduling Travel Visits Off-Campus
Travel forms can be filled out by any medical provider, but keep in mind that vaccines marked as required for your destination are necessary regardless of where you get your travel forms signed. In order to travel without obtaining a required immunization due to a medical contraindication, you will need to have your medical provider sign off on an Undergraduate Travel Vaccination Requirement Exemption Form.
Kaiser Patients:
You are welcome to get your required vaccines at Kaiser and have your forms signed off at SMC's Student Health Center, if that is easiest, or see a provider at Kaiser by completing an “International Travel” E-visit or calling 1-866-454-8855, Monday-Friday 8:30am-3pm, for a Travel consultation. No appointment is needed.
Preparing for Your In-Person Appointment
What to bring when you schedule an appointment:
- The Travel Health Form (see below for destination), with page 1 completed by the student.
- any documentation of additional vaccines obtained after speaking with our travel nurse
We highly encourage all students to utilize our email-started travel visit (see above), as this will expedite the process. Travel appointments will not be scheduled unless approved by the emailing travel nurse.
Cancellation and No-Show Policy
There is a $25 no-show charge for missed appointments.

LOCAL PHARMACIES TO GET REQUIRED IMMUNIZATIONS
Call to make sure these pharmacies have the vaccines you require, and whether you need to schedule an appointment or can walk in to get them. You can try scheduling online (it’s recommended you call first). If you have Kaiser insurance, you will need to get your vaccines through Kaiser. Those covered by other HMOs should check with their provider regarding where you are able to receive vaccines.
